Day 1

 

Morning Session

— Foundations of aligner biomechanics:
 

- Mechanical properties of aligner materials (PET-G, TPU, SmartTrack, TC-85)
- Stiffness, moment of inertia, span length and force delivery in thermoplastic appliances
- Stress–relaxation and clinical consequences
- Biological limits: capillary pressure thresholds and safe force levels.

Afternoon Session

— Limitations of tooth movements with aligners:
 

Predictability of different movements
Least predictable movements: rotations, extrusions, true intrusion
Why aligners overload teeth: setup increments, material thickness, stress–distribution problems.
 

— Anchorage issues with aligners:
 

Threshold forces, resistance levels, equilibrium analysis
T1 and T2 phase of treatment.
 

— Clinical consequences: refinement cycles, uncontrolled tipping, anchorage loss:
 

- When aligners fail and why: evidence-based discussion.

 

Day 2

 

Morning Session

— Clinical case analysis and effective treatment planning:
 

Extraction and non-extraction cases
Deep bite correction: what is achievable vs not
Expansion and asymmetrical corrections
Anchorage with TADs: direct, indirect, and hybrid mechanics
Sequencing movements to avoid loss of control
How to assess predictability before approving the setup.

Afternoon Session

— Hybrid approach for molar uprighting:
 

- Why aligners struggle
- Simple aesthetic appliances for controlled uprighting: cantilevers and alpha-beta springs. With or without TADs.
 

— Hybrid approach for deep bite correction:
 

- Evidence-based approach. Real intrusion with aligners
- How minimal fixed appliances can produce true incisal intrusion.
 

— Intrusions and extrusions:
 

- Biological limits, span length problems, and force delivery issues
- Aesthetic auxiliaries for predictable vertical movements.
 

— Space Closure:
 

- Why aligners often fail to deliver the right M/F ratio
- Using aesthetic fixed mechanics to close spaces before switching to aligners
- Anchorage reinforcement strategies.

 

Day 3

 

Morning Session 

— Molar uprighting mechanics, Step-by-step exercises on typodont:
 

Different low-visibility appliances for controlled uprighting
Anchorage management and force vector control.
 

— Deep bite correction setups:
 

Practical construction of appliances for true intrusion
Intrusion mechanics on typodont (with different devices)
Adjustments for vertical dimension control.

Afternoon Session 

— Vertical corrections. Intrusion and extrusion:
 

- Vertical mechanics with aesthetic auxiliaries
- Managing intrusions when aligners alone are insufficient
- Controlled extrusion exercises with various appliances.
 

— Space closure mechanics:
 

- Preventing tipping and maintaining M/F ratios
- Anchorage: heavy vs light approaches.
 

— Planning a hybrid therapy:
 

- Build the short initial fixed-applinace phase, followed by the aligner phase
- Selection of the appropriate appliance depending on the typodont scenario.

Creator of the software T3do and DMA for Orthodontic treatment planning and mechanics design.

Dr. Fiorelli obtained his medical, dental, and orthodontic postgraduate degrees at the University of Siena in Italy.

He has also co-published with Prof. Birte Melsen the “Biomechanics in Orthodontics” multimedia software.

His main professional interests are orthodontic biomechanics, use of computers within the orthodontic practice, segmented arch technique and adult orthodontic treatment.
